Volunteer Application & Screening Process

1. Fill out the Volunteer Application Form pdf file
   (To view/print these forms, you will need to download the free Adobe Acrobat Reader.)
     
2. Mail or Fax the completed form to:

3. Contact Lisa Conroy, OSU Faculty and 4-H Tech Wizard Program Coordinator, (503)725-2113, to make an appointment for an Interview, New Mentor Orientation, and Training (8 Hours). OSU Extension 4-H conducts a criminal history check and requests an FBI background check through Oregon Mentors.org on all potential volunteers.
    
4. Upon completion of the Application process, you will receive a letter confirming your acceptance and Welcome to the 4-H Family! This process can take up to, but should not exceed, 6 weeks - depending on the response time of your references.
    
5. You will begin receiving the 4-H Newsletter which is published quarterly.
